Getting started is simple — download our new patient registration form, fill it out completely, and drop it off at our office or mail it to us at 1526 Bridgewater Lane, Suite 100, Kingsport, TN 37660. Once we receive your form, our staff will review your information, match you with one of our providers, and contact you to schedule your first appointment. We look forward to having you as a patient!
We perform lab work right here in our office — no separate trip to an outside lab required. For physicals and follow-up visits, we also offer pre-visit lab draws so that your results are ready before your appointment. That way, your provider can go through everything with you in person during your visit rather than following up later by phone or portal message.
Office hours are 8:00 AM – 4:30 PM, Monday through Friday.
